Prelicensed Social Worker - Part time

Assess patients' psychosocial status, strengths, and areas of need affecting rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment. Participate in care planning with the patient and healthcare team to identify interventions that improve quality of life and meet treatment goals. Provide supportive counseling, educational goal-directed counseling for transplant seekers, and referrals to community resources including vocational rehabilitation and home health services. Monitor patient adjustment to dialysis, address non-adherence and quality of life concerns, and facilitate discussions regarding advance directives and DNR status with the medical team and family. Collaborate with insurance coordinators to address payment issues, review patient rights, and manage transplant waitlists. Contribute to monthly quality review meetings, weekly team huddles, and staff training on psychosocial topics while adhering to caseload guidelines and state regulatory requirements.
