Job Summary
Assess patients' psychosocial status and strengths to inform care planning and rehabilitation goals for in-center and home dialysis patients. Provide psychosocial counseling, education on treatment modalities, and support for transplant waitlist management while addressing barriers to adherence and quality of life. Collaborate with the interdisciplinary team to facilitate referrals for community resources, vocational rehabilitation, and insurance assistance. Participate in monthly quality review meetings, weekly team huddles, and process improvement activities to enhance patient outcomes. Maintain updated patient resource lists and assist in the interview process for new personnel as requested. Requires a Master's in Social Work with a state temporary license or provisional LCSW license.
Required Qualifications
- Masters in Social Work
- state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW License (if required by state)
- Obtains necessary state licensure to work independently without supervision within the first two (2) years or as required by state guidelines
- 0 – 2 years' related experience
