Prelicensed Social Worker - Part time

Assess patients' psychosocial status and strengths to inform care planning and rehabilitation goals for in-center and home dialysis patients. Deliver psychosocial counseling, education on kidney disease, and goal-directed support to improve quality of life and treatment adherence. Collaborate with the interdisciplinary team to address non-adherence, facilitate transplant referrals, and manage insurance and financial assistance issues. Participate in weekly team huddles, monthly quality review meetings, and process improvement activities to enhance patient outcomes. Provide staff training on psychosocial topics and assist with new personnel interviews. Requires a Master's in Social Work and a state temporary license; entry-level role with no supervision.
