Prelicensed Social Worker - Part time

Assess patients' psychosocial status and strengths to inform care planning and rehabilitation goals for in-center and home dialysis patients. Provide psychosocial counseling, education on chronic kidney disease, and support for transplant referrals while collaborating with the interdisciplinary team. Address quality of life concerns, insurance issues, and discharge requests, and facilitate access to community resources and vocational rehabilitation. Participate in weekly team huddles, monthly quality review meetings, and staff training on psychosocial topics. Maintain updated patient resource lists and adhere to state regulatory caseload guidelines. This entry-level MSW role supports the FMCNA commitment to quality indicators and patient satisfaction.
