Job Summary
Assess patients' psychosocial status and strengths to inform care planning for in-center and home dialysis patients. Provide supportive counseling, education on chronic kidney disease, and goal-directed guidance for transplant seekers. Collaborate with the interdisciplinary team to address non-adherence, facilitate referrals to community resources, and manage advance directives and DNR discussions. Monitor quality indicators related to missed treatments and quality of life trends during monthly reviews. Liaise with insurance coordinators to address payment issues and refer patients to billing solutions. Assist with hiring decisions, maintain updated patient resource lists, and participate in weekly team huddles. This entry-level MSW role requires a temporary license or provisional LCSW and focuses on improving rehabilitation and quality of life for patients.
Required Qualifications
- Masters in Social Work
- state temporary license
- Registered Social Worker Clinical Intern
- Associated Social Work license
- Provisional LCSW License
- 0 – 2 years' related experience
- state licensure to work independently without supervision within the first two (2) years or as required by state guidelines
- Travel required (if multiple facilities)
