Job Summary
Assess patients' psychosocial status and strengths to inform care planning with the interdisciplinary team. Provide psychosocial support, counseling, and education regarding dialysis, transplant, and End Stage Renal Disease to improve quality of life and adherence. Collaborate with insurance coordinators to address payment issues and refer patients to community resources, vocational rehabilitation, and advance directive discussions. Participate in monthly quality review meetings, weekly team huddles, and process improvement activities to support FMCNA Quality Goals. This entry-level MSW role requires a temporary license or provisional LCSW and 0–2 years of related experience.
Required Qualifications
