Job Summary
Assess patients' psychosocial status and strengths to inform care planning and rehabilitation goals. Provide supportive counseling, educational guidance on transplant processes, and referrals to community resources for optimal treatment outcomes. Monitor quality indicators related to adherence and participate in monthly review meetings with the interdisciplinary team. Address insurance and financial assistance inquiries while collaborating with coordinators on coverage changes. Assist with staff interviews, maintain updated patient resource lists, and contribute to weekly team huddles. This entry-level MSW role supports the FMCNA commitment to Quality Indicators and Outcomes, focusing on improving the quality of life for dialysis patients.
Required Qualifications
- Masters in Social Work
- Must have state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW License (if required by state)
- Obtains necessary state licensure to work independently without supervision within the first two (2) years or as required by state guidelines
- 0 – 2 years' related experience
