Job Summary
Assess patients' psychosocial status and strengths to inform care planning and identify effective interventions for rehabilitation and quality of life. Provide supportive counseling, educational goal-directed counseling for transplant seekers, and referrals to community resources including vocational rehabilitation and home health services. Monitor patient adherence to dialysis, address quality outcome concerns, and participate in monthly quality review meetings. Facilitate the transplant referral process, manage transplant waitlists, and provide education on End Stage Renal Disease support services. Collaborate with the interdisciplinary team on patient admission, discharge requests, and advance directive discussions, while maintaining updated patient resource lists and contributing to weekly team huddles. This entry-level MSW role supports the FMCNA commitment to Quality Indicators and Outcomes activities.
Required Qualifications
- Masters in Social Work
- Must have state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW License (if required by state)
- Obtains necessary state licensure to work independently without supervision within the first two (2) years or as required by state guidelines
- 0 – 2 years' related experience
