Job Summary
Entry-level MSW role providing psychosocial services to dialysis patients in collaboration with the interdisciplinary team. Responsibilities include patient assessment, care planning and counseling to address barriers to rehabilitation and quality of life; education and support to patients and families; facilitating transplant referrals; assisting with access to community resources and vocational rehabilitation; documentation of MSW interactions; participation in monthly Quality review meetings and adherence to social work policies; travel to multiple facilities as needed.
Required Qualifications
- Master of Social Work (MSW)
- Must have state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW License (if required by state)
