Job Summary
Conduct in-reach within correctional facilities to identify and engage justice-involved individuals eligible for Enhanced Care Management services prior to release. Receive referrals from Medi-Cal Managed Care Plans and partners to determine eligibility, then develop individualized pre-release care plans in collaboration with the Member and multidisciplinary care teams. Coordinate discharge planning by linking members to medical, behavioral health, substance use treatment, housing, and community-based services while assisting with appointment scheduling and reentry needs. Manage assigned caseloads by maintaining accurate records, tracking program data, and completing timely documentation in WestCare's Clinical Data System. Provide culturally responsive service coordination and follow-up to ensure service connection, performing duties across correctional facilities, office environments, and community settings with local travel.
Required Qualifications
- Valid driver's license
- Ability to obtain and maintain WestCare's vehicle insurance
- Ability to obtain and maintain successful background clearance
- CPR Certification
- First Aid Certification
- Annual Tuberculosis test
- High School diploma or equivalent
- Must complete required training related to CalAIM Justice-Involved services, documentation standards, and confidentiality requirements established by the California Department of Health Care Services and WestCare Foundation
- Local travel is required to correctional facilities, partner agencies, and community providers
Desired Qualifications
- Bilingual
- Two (2) years of case management, reentry services, outreach, or care coordination experience
- Experience working with justice-involved populations, behavioral health, or substance use treatment populations
- Graduation with a two-year degree in Addiction Counseling, Psychology, Social Work, Community and Human Services, Rehabilitation Counseling, or related field
- Strong knowledge of community resources, housing services, behavioral health services, and reentry supports
- Excellent communication and interpersonal skills
- Excellent documentation and organizational skills
- Ability to manage multiple priorities and maintain detailed records
- Excellent computer skills
