Job Summary
Practice Transformation Specialists act as HQ’s direct interface with providers to implement products and execute ACO initiatives, support wellness visits and transitions of care, and train office staff/providers on value-based care processes. The role emphasizes relationship-building with physicians and healthcare executives to improve financial and quality performance, while ensuring practices meet agreed transformation plans. Key duties include direct practice support, achieving performance goals, training and development in our Value Based Care Curriculum, and ongoing client success through listening to practice concerns and identifying opportunities for improvement. Requires Oregon residency, at least 3 years of EHR experience, 4–6 years in medical practice management, and strong communication, organizational and project management skills; travel to practices within southern Oregon (~25%) is expected.
Required Qualifications
- Graduate of an accredited college or university.
- 4-6 years of significant and relevant work experience in medical practice management in lieu of educational requirements may be accepted, particularly with significant administrative experience in a clinic setting.
- 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes
