Job Summary
The Practice Support Specialist serves on the Choices Practice Improvement Team to deliver site-based training, coaching, and support for high fidelity Wraparound and Care Coordination with Family Preservation families. Responsibilities include training staff, modeling reflective practice, providing process consultation on applying the practice model, coaching for ongoing skill development, acting as a consultant to improve practice skills across four phases (engagement, plan development, implementation, and sustainment), delivering quality assurance/improvement through data-driven measures, planning quarterly learning opportunities, colaborarating with site leadership and corporate leadership to support staff development, and potentially carrying a partial caseload to model opportunities. Candidates should have a Bachelor’s degree in Social Work or related field, 2+ years in care coordination or supervisory roles, experience in training/consultation, strong communication and writing abilities, and a valid driver’s license.
Required Qualifications
- Minimum of Bachelor’s degree in Social Work or related human service field is required in most contracts
- 2 years of experience in a care coordination role or supervisory role
- Experience in providing training and/or consultation within the human services field
- Strong knowledge of youth and adults with mental/behavioral health issues
- Strong communication and writing skills
- Demonstrated ability to work effectively with internal and external professionals and as part of a team
- Valid driver’s license in state of residence
