Practice Liaison

The Practice Liaison will serve as the primary point of contact between Diverge Health and partnered practices, leading onboarding, trainings, and ongoing collaboration to optimize use of Diverge Health technology and care solutions. Responsibilities include developing onboarding plans, delivering trainings on value-based care and the company’s incentive plan, translating performance data (scorecards, claims data, gap reports) into actionable steps, coordinating with practice leadership to drive value-based transformation, facilitating access to resources (Medical Directors, Care Team, technology), maintaining practice profiles in the CRM, and supporting travel and administrative duties to meet engagement and performance goals. The role requires a Bachelor’s degree in a related field, 2–4 years in healthcare operations or customer-facing roles, familiarity with primary care workflows, knowledge of HEDIS measures and risk adjustment methods, strong communication/presentation skills, and comfort with change in a high-growth environment. Travel within the community is required; valid driver’s license and reliable transportation are needed. This is a full-time, exempt, salaried position with a base salary range of $90,000-$105,000.
