Job Summary
Outpatient geriatric medicine physician needed to provide comprehensive geriatric assessments and consultative care for older adults; collaborate with inpatient geriatrics and community primary care providers; deliver care in the outpatient clinic with occasional inpatient coverage; extend visit times for goals of care, advance care planning, and shared decision-making; work with a multidisciplinary geriatric team to optimize outcomes; participate in teaching and mentoring fellows and potentially Family Medicine residents; play a key role in building and anchoring the hospital's Center for Successful Aging with leadership and innovation opportunities; practice emphasizes value-based, patient-centered, team-based care across clinic, home, and assisted living settings.
Required Qualifications
- MD or DO with board certification/eligibility in Geriatric Medicine (Internal Medicine or Family Medicine trained)
- Strong clinical skills in geriatric medicine
- Commitment to value-based, patient-centered care models
- Excellent communication and interpersonal skills
- Interest in program development and innovation in Geriatric Medicine
