Post Transplant Coordinator ( RN )

Coordinate peri-operative and post-transplant care as a registered nurse for the Post-Transplant Coordinator role at Houston Methodist. Collaborate with the interprofessional transplant team to ensure coordination of care for post-transplant patients, provide patient and family education, monitor patient progress, and document care in alignment with OPTN/UNOS policies, CMS requirements, and department goals. Demonstrate clinical knowledge in transplant care, problem-solve complex caseloads, and communicate effectively with patients, families, and healthcare providers to support safe, high-quality outcomes. Requires a Bachelor’s degree or higher in Nursing, Texas RN licensure, and BLS certification with approximately four years of nursing experience; on-site duties and responsibilities are conducted in Houston, TX.
