Job Summary
Post-Acute Clinical Resource Specialist manages and verifies post-acute transition plans for patients exiting acute care. Coordinates placements with skilled nursing facilities, rehabilitation centers, home health agencies, and other post-acute providers; collaborates with inpatient clinicians, community-based providers, physicians, nurses, social workers, and case managers to ensure seamless care coordination; communicates with insurance payers to obtain authorizations and verify coverage; maintains detailed documentation of all transitions and ensures compliance with regulatory standards and organizational policies; role supports day shift with some weekends in Traverse City, MI; required high school diploma and at least 2 years in a hospital/medical-related field, plus immunization requirements.
Required Qualifications
- High school diploma or equivalent
- 2 years of an equivalent combination of education and experience in a hospital/medical related field
- Vaccination against Measles, Mumps, Rubella and Varicella or approved exemption; flu vaccine during flu season or exemption
