Job Summary
Population Health RN in a Rural Health Clinic, onsite. Focuses on improving patient outcomes through proactive care management, chronic disease education, care coordination, and patient engagement to reduce preventable complications and avoid unnecessary ER visits and hospitalizations. Works with providers, care teams, patients, and community resources to deliver high-quality, comprehensive care. Responsibilities include proactive population health management, patient education, and coordination across care teams to empower patients to self-manage their health conditions. Requires WA RN license and BLS; additional certifications welcomed. Bonus opportunities include sign-on and retention bonuses.
Required Qualifications
- Current RN license to practice in Washington State
- current BLS certification
- Willing to complete additional certification as requested
