Job Summary
Population Health Nutrition Specialist collaborates with programmatic and clinical colleagues to respond to value-based payment models in Hawaii. The role assesses nutrition and diet needs of patients at high risk of chronic disease, providing clinical nutrition counseling in a culturally sensitive manner, and addresses gaps in care related to food, nutrition, and diet with consideration of social determinants of health such as food insecurity and access. Focus areas include preventive strategies, disease intervention, and reducing avoidable costs by promoting health education, disease prevention, and primary care within a culturally sensitive healthcare home environment.
Required Qualifications
- Registered Dietitian with current CDR Registration or RD eligible
- Licensed Dietitian in Hawaii
- Master’s Degree in Nutrition (or coursework supplemented by public health courses)
- Telehealth experience (preferred)
- Experience in health services for chronically ill patients desirable
- Proficient in word processing, data entry, spreadsheets, internet, email, and electronic health records
- Strong verbal and written communication
- Ability to partner with community health organizations
- OSHA training annually
- BCLS every two years
- Hawaii Driver’s License strongly desired
- Ability to work flexible hours
