Job Summary

Population Health Manager to lead and mentor a team in driving value-based care initiatives across diverse patient populations. Responsibilities include analyzing population health and utilization data to identify trends and care gaps, translating findings into actionable improvement strategies to enhance quality metrics and risk stratification, and collaborating with clinical and administrative teams to implement population health programs that improve chronic disease management, preventive screening rates, and patient engagement. The role supports performance in value-based programs (e.g., HEDIS, Medicare Advantage, Medi-Cal), acts as a liaison across departments to align initiatives with clinical operations and quality improvement efforts, and tracks KPIs related to screening and access to preventive care. The position requires ensuring compliance with FQHC guidelines and federal/state regulations, overseeing outreach and care gap closure workflows, and performing other duties as assigned. A Bachelor’s degree is required, with Master’s preferred, and 3-5 years of relevant experience, along with strong leadership, communication, and data-analysis capabilities.

Required Qualifications

  • Bachelor’s degree in Public Health, Health Administration, Nursing, or a related field (Master’s preferred)
  • 3-5 years of experience in population health, public health, or care coordination
  • Experience with data analysis and reporting; proficiency in population health management software and EHR systems
  • Strong leadership skills with experience managing teams
  • Excellent communication and presentation skills
  • Ability to prioritize, organize, and manage multiple projects in a fast-paced environment
  • Knowledge of federal and state health care regulations, FQHC guidelines, and population health best practices is a plus
  • Must be able to fluently speak, read and write English
  • Bilingual in Chinese or Spanish is a plus
