Job Summary
Oversee day-to-day operations of the Care Coordination Team, ensuring efficient service delivery and alignment with population health goals. Provide administrative oversight to nursing, LNA, and MA staff while developing workflows to support referral management and care transitions. Establish and monitor key performance indicators related to access, timeliness, and quality outcomes, leading continuous quality improvement initiatives focused on patient results. Facilitate strong internal collaboration across clinical and behavioral health teams while building relationships with community partners, hospitals, and payers to strengthen referral networks. This role supports nearly 4,000 community members through targeted programs in Caledonia, Essex, and Orleans counties, enhancing the ability of individuals to improve their lives.
Required Qualifications
- Bachelor's degree in Health Administration, Nursing Public Health, Human Services, or a related field
- Excellent communication, leadership, and change-management skills
