Job Summary
Conduct nutrition counseling, diabetes education, weight management strategies, and meal planning for patients within the Food Rx Program. Collaborate with Community Health Advocates across four Baltimore region hospitals and providers in the Collaborative Care Program outpatient clinic to identify eligible patients, coordinate enrollment, and provide MNT/DSME consults. Incorporate evidence-based strategies into population health management for patients with complex medical and social needs, particularly those facing food insecurity and uncontrolled diabetes. Report to the Regional Clinical Manager for Food Rx. Work flexible PRN schedule for 2-3 days a week.
Required Qualifications
- Bachelor's degree in nutrition/dietetics
- Master's degree
- Registered Dietitian in state of Maryland
- eligibility to obtain MD licensure
- CDR registration
