Job Summary
Conduct comprehensive psychiatric evaluations, manage psychotropic medications, and develop evidence-based treatment plans for geriatric patients in long-term care settings. Collaborate with interdisciplinary teams and case managers to create refined care plans, while participating in regular meetings to evaluate referrals and adjust strategies based on patient progress. Contribute to quality initiatives including performance improvement programs and medication reviews to optimize outcomes. Travel up to 50% of the time to meet healthcare partners across clinical settings in the Jamestown, NY to Erie area. Report to the Chief Medical Officer.
Required Qualifications
- Active, unrestricted New York PMHNP license
- Current and unrestricted PMHNP license in the state of PA and NY
- Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
- Active and unrestricted DEA (Drug Enforcement Administration)
- Master's or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health
- Proficiency in psychiatric evaluations, psychotropic medication management, and evidence-based treatment planning
- Strong ability to build relationships and work collaboratively with interdisciplinary teams in diverse care settings
- Awareness of social determinants of health and access to resources to address holistic patient needs
- Comfortable using electronic health records (EHRs), telehealth platforms, and other digital tools to enhance care delivery
- Willingness to travel within assigned regions to provide direct care and support to partner organizations, ensuring accessibility and continuity of services
Desired Qualifications
- Pennsylvania PMHNP license preferred, or willingness to obtain a Pennsylvania license
