Job Summary
Provide ministry-funded, community-based physiotherapy services within clients’ homes and/or through virtual service delivery. Conduct comprehensive assessments across geriatrics, neurology, orthopedics, and palliative care; develop and implement goal-oriented, evidence-based treatment plans in collaboration with clients and caregivers; assign, supervise, and monitor physiotherapy assistants when appropriate; educate clients and caregivers and connect them to community resources. Maintain accurate documentation including assessments, care plans, progress and discharge summaries aligned with referral-source requirements and College of Physiotherapists of Ontario standards. Requires registration as a physiotherapist in Ontario and a valid G2/G driver's license with reliable vehicle for community travel. Independent contractor with flexible scheduling (part-time or full-time) and per-visit remuneration; opportunity to provide in-person home visits or virtual assessments; new graduates welcome; strong clinical reasoning, leadership, and collaboration within an inter-professional team.
Required Qualifications
- Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario (required)
- Valid G2 or G driver’s license with access to a reliable vehicle for community travel
