Job Summary
Provide ministry-funded, community-based physiotherapy services within clients’ homes and/or through virtual service delivery. Complete comprehensive physiotherapy assessments using interviews, standardized tools, and informal assessment techniques across geriatrics, neurology, orthopedics, and palliative care. Develop and implement goal-oriented, evidence-based treatment plans in collaboration with clients and caregivers. Assign, supervise, and monitor physiotherapy assistants when appropriate. Provide education to clients and caregivers and connect them to appropriate community resources. Maintain accurate, timely documentation including assessments, care plans, progress and discharge summaries aligned with referral source requirements and College of Physiotherapists of Ontario standards. Independent practice with an opportunity to manage a hybrid caseload and consider virtual visits, supported by an extensive mentorship and professional development environment.
Required Qualifications
- Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario
- Valid G2 or G driver's license with access to a reliable vehicle for community travel
- Ability to work independently while collaborating within an inter-professional team
- Strong organizational skills, including self-scheduling, prioritization, and meeting reporting deadlines
- Excellent verbal, written, and report-writing communication skills
- Proficiency with computer applications, including MS Office Suite and proprietary clinical software
