Job Summary
Assess patients' functional needs, design individualized treatment plans, and implement physical therapy programs to maximize progress toward functional goals. Provide direct patient care, conduct thorough evaluations, and adjust care plans based on outcomes while consulting with other specialists. Direct support staff to achieve residents' functional goals and document all evaluations, treatment goals, and plans. Maintain positive relationships with ancillary staff, adhere to infection control and safety protocols, and demonstrate the ability to remain calm during emergency situations.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current license from the state Board of Physical Therapy Examiners
- At least one year of experience as a physical therapy aide
- Demonstrates the ability to read, write, and speak English sufficient to fulfill job requirements
- Demonstrates the ability to make independent decisions, solve practical problems that may or may not include a variety of concrete variables
- Functions independently with integrity, flexibility and works well with residents, personnel and support agencies
- Demonstrates knowledge of Employee Handbook and complies with the policies in the Handbook
- Complies with community policies and procedures for workplace safety including infection control procedures, emergency response and safety protocol
- Complies with all company policies and procedures related to resident rights; and resident confidentiality (HIPPA)
