Job Summary
Conduct individualized physical therapy programs including assessment, treatment, planning, implementation, education, and communication to maximize patient progress toward functional goals. Design and implement care plans based on thorough assessments, directing support staff to achieve residents' functional goals while consulting with other health professionals. Evaluate functional needs and outcomes, documenting evaluations, treatment goals, and plans in compliance with hospital policies, safety standards, and infection control protocols. Maintain positive working relationships with ancillary staff and act as a resource to promote physical therapy in the community.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current license from the state Board of Physical Therapy Examiners
- At least one year of experience as a physical therapy aide
- Demonstrates the ability to read, write, and speak English sufficient to fulfill job requirements
- Demonstrates the ability to make independent decisions, solve practical problems that may or may not include a variety of concrete variables
- Functions independently with integrity, flexibility and works well with residents, personnel and support agencies
- Ability to remain calm in emergency or crisis situations
- Demonstrates knowledge of Employee Handbook and complies with the policies in the Handbook
- Demonstrates the ability to read, write, and speak English sufficient to fulfill job requirements
