Job Summary
Assess, treat, and plan individualized physical therapy programs to maximize patient functional goals. Conduct thorough evaluations of residents, design care plans incorporating significant others, and direct support staff to achieve outcomes. Maintain positive relationships with health professionals, document evaluations and treatment goals, and ensure compliance with hospital policies, infection control standards, and resident rights. Provide leadership to staff, act as a community resource for physical therapy, and make independent decisions while adhering to safety protocols and HIPAA regulations.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current license from the state Board of Physical Therapy Examiners
- At least one year of experience as a physical therapy aide
- Demonstrates the ability to read, write, and speak English sufficient to fulfill job requirements
- Demonstrates the ability to make independent decisions, solve practical problems that may or may not include a variety of concrete variables
- Functions independently with integrity, flexibility and works well with residents, personnel and support agencies
- Demonstrates knowledge of Employee Handbook and complies with the policies in the Handbook
- Complies with community policies and procedures for workplace safety including infection control procedures, emergency response and safety protocol
- Complies with all company policies and procedures related to resident rights; and resident confidentiality (HIPPA)
