Job Summary
Conduct individualized physical therapy programs including assessment, treatment planning, implementation, and education to maximize patient functional goals. Evaluate residents' needs, adjust care plans, and direct support staff to achieve specific outcomes while maintaining positive relationships with other health professionals. Provide direction and leadership to ancillary staff, ensuring compliance with hospital policies, infection control standards, and safety protocols. Document evaluations, treatment goals, and plans accurately. Requires current APTA-approved license, state board certification, and at least one year of experience as a physical therapy aide.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current license from the state Board of Physical Therapy Examiners
- At least one year of experience as a physical therapy aide
- Demonstrates the ability to read, write, and speak English sufficient to fulfill job requirements
- Demonstrates the ability to make independent decisions, solve practical problems that may or may not include a variety of concrete variables
- Functions independently with integrity, flexibility and works well with residents, personnel and support agencies
- Demonstrates knowledge of Employee Handbook and complies with the policies in the Handbook
- Complies with community policies and procedures for workplace safety including infection control procedures, emergency response and safety protocol
- Complies with all company policies and procedures related to resident rights; and resident confidentiality (HIPPA)
