Job Summary
Provide high-quality community-based physiotherapy services including comprehensive assessment, treatment planning, implementation, and evaluation in home and clinic settings. Collaborate with a multidisciplinary team of Allied Health Professionals, GP’s, and referrers to ensure optimal patient care; produce initial, progress, and final reports as well as treatment management plans that meet funding requirements (NDIS, DVA, Medicare). Build person-centred care plans, sustain relationships with Case Managers and community partners, and travel within the Ringwood area to deliver services. Maintain professional practice values of care, respect, integrity, and service excellence while supporting the Lifecare/Homecare team and aspiring to career growth within the APM Group.
Required Qualifications
- Physiotherapy degree with General AHPRA registration
- Ability to work within a multi-disciplinary team and autonomously in the community
- Enjoyment of working with older adult and adult disability demographics
- Full working rights in Australia
- Clean driver licence with access to own vehicle and comprehensive car insurance
- Current Working with Children Check and NDIS Workers Screening Clearance, or able to obtain
- Willing to undergo a National Criminal History check upon successful application
