Job Summary
Provide in-person medical care to patients in assigned skilled nursing and rehabilitation facilities, performing assessments, managing treatment plans, and updating EMR documentation. Lead or collaborate with facility teams to ensure quality outcomes, reduce hospital readmissions, and guide patients through transitional care. Serve as a liaison between Trumen and facility leadership while participating in value-based care programs. Maintain availability during regular business hours on scheduled days. This independent contractor role offers flexible scheduling with support for call coverage and monthly medical education.
Required Qualifications
- MD or DO
- current, unrestricted Texas medical license
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Comfortable practicing independently in skilled nursing facilities
- Strong clinical judgment

- Reliable transportation within the Southwest Houston area
Desired Qualifications
- 1+ year of experience in post-acute, geriatric, or SNF care
