Physician, Skilled Nursing Facility (SNFist) (Sacramento, CA)

Skilled Nursing Facility Physician to provide comprehensive medical assessments and ongoing management for patients in SNFs, focusing on post-acute, rehabilitative, and long-term care. Responsibilities include daily rounds, coordinating care with nursing, rehabilitation, social work, and case management teams; communicating with families about goals and plans; ensuring compliance with SNF regulations and quality measures; collaborating with hospitalists, PCPs, and specialists to support smooth transitions of care and reduce readmissions; participating in quality improvement, population health, and value-based care initiatives; and maintaining timely EMR documentation. Requires MD/DO, board certification in Internal Medicine or Family Medicine (geriatric certification preferred), active CA license, and experience in geriatric/post-acute settings. Preferred fellowship in Geriatric Medicine or post-acute/rehab experience; knowledge of CMS SNF regulations and billing; PSLF eligibility; sign-on and relocation benefits; strong interdisciplinary teamwork and patient-centered approach.
