Job Summary
Skilled Nursing Facility Physician to provide comprehensive medical assessments and ongoing management for patients admitted to skilled nursing facilities, focusing on post-acute, rehabilitative, and long-term care. Responsible for managing medically complex, multi-morbid patients, conducting daily or scheduled rounds, coordinating with interdisciplinary teams, communicating with families and staff, ensuring regulatory compliance and quality measures, participating in care plan meetings and quality improvement initiatives, and supporting transitions of care to reduce readmissions. Requires MD/DO, board certification in Internal Medicine or Family Medicine (geriatric certification preferred), active CA license, and demonstrated post-acute/geriatric experience. Proficiency in EMR and collaborative, patient-centered care in an interdisciplinary facility-based environment. Preferred fellowship in Geriatric Medicine or experience in post-acute/rehabilitation medicine; knowledge of CMS regulations, SNF billing/documentation, and value-based care programs.
Required Qualifications
- Medical degree (MD or DO) from an accredited school of medicine
- Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred)
- Active and unrestricted medical license in California
- Demonstrated experience in post-acute, skilled nursing, long-term care, or geriatric settings
- Strong clinical skills in managing complex, multi-morbid patients
- Excellent communication and interpersonal skills, with a patient- and team-centered approach
- Ability to work effectively in an interdisciplinary, facility-based environment
- Proficiency in electronic medical records (EMR) systems
