Job Summary
Physician needed to deliver high-quality, patient-centered care through in-home or virtual health evaluations as part of Signify Health’s Healthcare & Medical Services team. Required Qualifications
- Physician
- Doctorate or professional degree in medicine
- Board-certified or eligible in relevant specialty
- Ability to deliver in-home and virtual health evaluations
- Strong documentation of clinical findings and care plan development
- Collaboration with interdisciplinary care teams
- Familiarity with value-based care models
- Proficiency with analytics and healthcare technology tools
