Required Qualifications
- MD or DO license in good standing
- Board-certified/board-eligible in relevant specialty (e.g., Family Medicine, Internal Medicine)
- Ability to deliver in-home and virtual health evaluations
- Experience documenting clinical findings and developing care plans
- Familiarity with value-based care models and analytics
- Strong collaboration with interdisciplinary care teams
- Excellent communication and patient-centered care skills
- Proficient in electronic health records and healthcare technology
