Job Summary
Physician Services Clinical Coordinator to serve as the central connection between providers, patients, caregivers, and healthcare partners. Coordinate services, facilitate communication among care teams, manage patient needs and follow-up activities, and help patients and families access resources to support health and well-being. Focus on elderly and medically complex patients with strong organization, communication, and problem-solving skills to ensure care plans are carried out smoothly and efficiently.
Required Qualifications
- High school diploma or GED
- CENA certification, MA certification, or equivalent work experience
- Minimum 1 year of experience in medical field or medical office setting
- Strong grasp of medical terminology
- Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint)
