Job Summary
Conduct comprehensive geriatric primary care and post-acute care services, including independently completing patient histories, physical exams, and appropriate diagnoses. Order and interpret diagnostic tests, prescribe pharmacologic and non-pharmacologic treatments, and collaborate with Advance Practice Providers to manage acute and long-term medical needs. Maintain patient records, educate patients and families on treatment instructions, and evaluate procedures to improve service delivery and profitability. Requires a valid MD or DO license, Board Certification in Internal Medicine or Family Medicine, and 2+ years of experience in geriatric care.
Required Qualifications
- Doctorate degree
- Valid Doctor of Osteopathic Medicine (DO) or Medical Doctor (MD) license
- 2 or more years of experience in Geriatric Care, Primary Care, or Post Acute
- Valid Medical Doctor (MD) certification/license in Pennsylvania (PA)
- Valid BC/BE Internal Medicine or Family Medicine
- Current DPS Certification or DPS In-Progress
- Current DEA Certification or DEA In-Progress
- Current unrestricted license to practice medicine in the State of employment
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine
Desired Qualifications
- Experience in Value Based Care
- Previous experience in EHR documentation
- Proficiency in documenting and billing in various healthcare settings
- A positive attitude, a commitment to delivering quality care, and an interest in joining a team-first approach among providers
- Previous experience in EHR documentation
- Experience or a passion for value-based care
- Senior living, SNF, IPR, and LTACH experience
