Job Summary
Provide comprehensive primary care services to patients of all ages in an outpatient, team-based setting; manage chronic conditions and develop personalized treatment plans; perform preventative care, wellness visits, and routine screenings; collaborate with nurse practitioners, specialists, and care teams to optimize patient outcomes; maintain accurate electronic health records; participate in quality improvement initiatives to enhance patient care and clinic operations; engage in community outreach and health education as applicable.
Required Qualifications
- Board Certification or Eligibility in Family Medicine or Internal Medicine or Med Peds
- Strong clinical and diagnostic skills to provide high-quality primary care
- Commitment to patient-centered care and the ability to build lasting relationships with patients and families
- Excellent communication skills and a collaborative mindset for working within a team-based care model
- Willingness to work some early morning, evening, and rotating weekend shifts to increase patient access
- Experience in outpatient primary care (preferred, but not required for new graduates)
