Job Summary
Provide comprehensive primary care services to patients of all ages, including managing chronic conditions, preventive care and wellness visits, and routine screenings. Collaborate with nurse practitioners, specialists, and care teams to optimize patient outcomes; maintain accurate and timely electronic health records; participate in quality improvement initiatives; engage in community outreach and health education efforts as applicable.
Required Qualifications
- Board Certification or Eligibility in Family Medicine or Internal Medicine or Med Peds
- Strong clinical and diagnostic skills to provide high-quality primary care
- Commitment to patient-centered care and the ability to build lasting relationships with patients and families
- Excellent communication skills and a collaborative mindset for working within a team-based care model
- Desire to work in a community-focused practice and contribute to a positive workplace culture
- Willingness to work some early morning, evening, and rotating weekend shifts to increase patient access
- Experience in outpatient primary care (preferred, but not required for new graduates)
