Physician - Primary Care

Provide primary care to non-surgical adult patients within a VA Community Based Outpatient Clinic in Oakland, CA. In a Patient Aligned Care Team, the Physician assembles with a team (PCP, RN, LVN, Clerk) to manage a patient panel (800-1200). Responsibilities include evaluating, diagnosing, treating, and consulting for diverse adult conditions; stabilizing and dispositioning emergent cases per medical staff policy; collaborating with mid-level providers and other specialties to co-manage patient care; managing chronic diseases (e.g., diabetes, hypertension, COPD, chronic pain) and providing preventive health services in a team-based setting; additional duties as assigned.
