Job Summary
Physician/Medical Director leading population-health oriented care within Eventus WholeHealth’s Outcomes-Centered Healthcare model. Provides clinical leadership and collaboration with Advanced Practice Clinicians to deliver high-quality, team-based care across assigned facilities, with potential Medical Directorship responsibilities for clinical oversight. Requires MD/DO with state medical license and board certification in Internal Medicine, Family Medicine, or Geriatrics (CMD certification a plus if serving as Medical Director). Strong emphasis on interdisciplinary collaboration, care coordination, EMR familiarity (AthenaHealth preferred), and population-health strategy in post-acute and long-term care settings. Required Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) required
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
- CMD certification a plus; required within 2 years if serving in a Medical Directorship capacity
- Active and unrestricted medical license in applicable state(s)
- DEA registration as required
- Minimum 2–3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred
- Experience collaborating with interdisciplinary teams and facility leadership
- Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow
