Job Summary
Attending Physician/Medical Director responsible for population-health–driven, team-based care across assigned skilled nursing and assisted living facilities. Leads clinical care within Eventus WholeHealth’s Outcomes-Centered Healthcare model, collaborates with Advanced Practice Clinicians, and may assume Medical Directorship duties for facility(s). Emphasis on high-quality, accountable care, interdisciplinary collaboration, and proactive care plans; supports AI-enabled clinical decision tools, care coordination workflows, and ongoing professional development. Qualified candidates hold an MD or DO, with board certification in Internal Medicine, Family Medicine, or Geriatrics preferred, an active license, and CMD certification if serving as Medical Director. Required Qualifications
- MD or DO required
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
- Active and unrestricted medical license in applicable state(s)
- DEA registration as required
- CMD certification a plus; required within 2 years if serving in a Medical Directorship capacity
- Minimum 2–3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred
- Experience collaborating with interdisciplinary teams and facility leadership
- Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow
