Job Summary
Provide primary care services including diagnosis, treatment of chronic and acute illness, and preventative care in collaboration with the Chief Medical Director. Conduct health assessments and evaluate patient needs across the clinic, PACE Day Center, and patient homes. Serve as a member of the interdisciplinary team to develop and implement comprehensive care plans, making appropriate referrals to contracted community providers. This role supports the Program of All-inclusive Care for the Elderly, balancing medical, emotional, physical, and social needs of seniors to help them live safely at home. Physicians must hold a Colorado license and possess experience with medically complex populations.
Required Qualifications
- Licensed M.D. or D.O. in the state of Colorado
- Controlled substance license with current DEA license number or ability to obtain
- Eligible or currently possesses Medicare/Medicaid UPIN number
- Valid Colorado driver's license, adequate automobile insurance and access to reliable transportation maintained throughout employment
- Strong clinical reasoning, diagnostic, and medical decision-making skills
- Knowledge of geriatric medicine, chronic disease management, preventative care and evidence-based practices
- Ability to lead and collaborate effectively
Desired Qualifications
- Board Certification or eligibility in Family Medicine, Internal Medicine, or another related specialty preferred
- Three (3) years of physician experience preferred
- Experience working with older adults, medically complex populations or multiple chronic conditions preferred
- Experience in geriatrics, primary care, hospice, palliative care, home-based care, or PACE preferred
