Job Summary
Provide direct patient care, including preventive care, diagnosis, treatment, and management of acute and chronic conditions. Develop individualized treatment plans, educate patients on health maintenance, and collaborate with a multidisciplinary team to coordinate care. Maintain accurate electronic health records in compliance with federal, state, and organizational standards while participating in quality improvement initiatives and peer reviews. Support community outreach, health education, and patient engagement initiatives tailored to a diverse population. Adhere to all FQHC regulatory requirements, including HRSA, HIPAA, and OSHA guidelines. Supervise and mentor advanced practice providers as needed. Required Qualifications
- Medical degree (MD or DO) from an accredited institution
- Board certification or board eligibility in Family Medicine, Internal Medicine, or Pediatrics
- Valid and unrestricted California medical license
- DEA registration and ability to obtain additional credentials as required
Desired Qualifications
- Experience working in a community health or underserved setting preferred
- Strong commitment to health equity and serving diverse populations
- Proficiency in electronic health records (EHR) and familiarity with FQHC documentation standards
- Bilingual proficiency (Spanish or other languages) is a plus
