Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members to identify care barriers and facilitate referrals. Complete assessments using provided patient-specific forms without prescribing medication, ordering labs, or altering treatment regimens. Earn up to $5000 weekly with an average of 6-9 daily visits, utilizing a pay-per-test model for eligible diagnostic services. Requires a state license and Medicaid enrollment willingness; shifts available Monday through Sunday with flexible scheduling. Independent contractor status with malpractice coverage and travel support including airfare and hotel.
