Physician (MD/DO) for In-home Visits

Contract physician conducts in-home health evaluations for health-plan members (Medicare Advantage/Medicaid; some commercial plans) as an independent contractor. Responsibilities include in-home visits focused on overall health and barriers to care, reviewing medical history and current medications, performing physical evaluations in the patient’s home, and coordinating with health plans. Pre-populated forms are provided; physicians do not prescribe medications or alter treatment regimens. State licensure required; Medicaid license enrollment supported in some states. Compensation is per-visit with potential multipliers and optional pay-per-test incentives; shifts are available Mon-Sun with a flexible schedule. Location: Thorntown, IN; malpractice coverage provided; travel-related expenses (airfare, hotel, rental car) may apply based on eligibility.
