Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members using provided patient-specific forms without prescribing medicine, ordering lab tests, or altering treatment regimens. Identify barriers to care and facilitate referrals to primary care physicians while encouraging family participation. Work flexible schedules Monday through Sunday, earning up to $5,000 weekly based on an average of 6-9 daily visits. Requires a state license and willingness to obtain a Medicaid license; independent contractor status applies.
