Physician (MD/DO) contractor for in-home health evaluations visiting health plan members in homes across KY. Responsibilities include conducting comprehensive in-home assessments focusing on overall health and barriers to care, reviewing medical history, performing physical evaluations, and coordinating with health plans. Activities may include pre-populated patient forms, consideration of Medicaid/license requirements, and potential participation in additional diagnostic/preventive tests under pay-per-test models. Flexible scheduling with availability Monday–Sunday, malpractice coverage, and 1099 contractor status are provided. Average earnings reflect base pay per completed assessment plus incentives, with visits conducted in patients’ homes. Requires state license; Medicaid license enrollment supported where applicable.
