Job Summary
Manage chronic conditions for a small panel and lead high-risk huddles with clinical oversight of Advanced Practice Providers. Act as on-site supervising physician for assigned APPs, providing complex patient visits and conducting monthly chart audits. Collaborate with community health workers, nurse navigators, and care managers to build team culture, while frequently reviewing quality metrics and clinician productivity. Sign DME orders, death certificates, and other regulated orders on behalf of APPs. Assist with service inquiries, compliance resolutions, and performance reviews for direct reports. Participate in clinical education on diagnosis and chronic condition management, and work with the Regional Medical Director to identify care improvement opportunities.
Required Qualifications
- Bachelor's Degree
- Must be a Doctor of Medicine or Osteopathy
- Must maintain a valid driver's license and maintain a good driving record
- Active CPR Certification
- Unrestricted DEA
- Completion of an accredited Residency Program
- Unrestricted Medical License
- Board certification is required for all newly hired or promoted on or after January 1, 2026; must obtain and maintain certification in accordance with organizational and applicable regulatory standards
Desired Qualifications
- 3 years of experience as a Physician collaborating with Advance Practice Providers
- Experience in Geriatrics or complex patients
