Job Summary
Lead clinical performance for a geriatric primary care pod by supervising assigned APPs and physicians, conducting chart audits, and performing complex patient visits. Collaborate with community health workers and nurse navigators to drive care management, while leading high-risk huddles to ensure documented plans of care. Review quality metrics and clinician productivity to improve HEDIS performance, and partner with the Regional Medical Director on care improvement initiatives. Manage team dynamics through performance reviews and discipline actions, and sign DME orders and death certificates as regulated. Participate in clinical education on chronic conditions and assist with service inquiries and patient complaints. Requires a medical license, residency, and board certification; includes a $50k retention bonus.
Required Qualifications
- Bachelor's Degree
- Must be a Doctor of Medicine or Osteopathy
- Must maintain a valid driver's license and maintain a good driving record
- Active CPR Certification
- Unrestricted DEA
- Completion of an accredited Residency Program
- Unrestricted Medical License
- Board certification is required for all newly hired or promoted on or after January 1, 2026; must obtain and maintain certification in accordance with organizational and applicable regulatory standards
Desired Qualifications
- 3 years of experience as a Physician collaborating with Advance Practice Providers
- Experience in Geriatrics or complex patients
