Provide direct patient care in accordance with evidence-based treatment guidelines within a Patient Centered Medical Home model, delivering well care and managing chronic disease states. Interact with patients and families to keep them informed and engaged in health care decisions while providing counseling education on wellness, prevention, and health maintenance. Refer patients to specialty care services and educational training as appropriate. Utilize disease registries to identify care gaps and work with care managers to improve healthcare metrics. Maintain accurate documentation of patient encounter records to meet CMS, HIPAA, and governmental regulations. Participate in process improvement to enhance access to care, including same-day appointments and extended hours. Collaborate with Practice Development and Improvement Managers to achieve Patient Centered Medical Home status and NCQA certification. Act in a fiscally responsible manner to deliver value through quality and cost efficiency.
